Electronic Visit Verification is a federal requirement (Cures Act §12006) for Medicaid personal care and home health visits. Every visit — no matter which system your agency uses day to day — has to make it into HHAeXchange (HHAX), Minnesota's EVV aggregator.
Second consecutive quarter below threshold? DHS requires a Corrective Action Plan — due about a month after the notice, identifying what you'll fix, what specific actions you'll take, and how you'll monitor compliance going forward. Miss that window, or stay below threshold, and DHS can move to payment holds or termination of enrollment.
